Neelyville School Climate Surveys for Staff, Students, and Families
As we strive to ensure our school provides a safe, supportive, and rigorous learning environment for our students, we want to learn more about the experiences of our school community. This year, we invite our staff, students, and families to participate in the Neelvyille R-IV School Climate and Culture Surveys as directed by the Missouri Department of Education.
Staff Survey
Deadline September 11, 2026
The staff survey, which will take approximately 15 minutes to complete, includes questions about school safety, student achievement, and your perceptions of the school environment.
Parent/Guardian Survey
Deadline September 11, 2026
The parent/guardian/community survey, which will take approximately 10 minutes to complete, includes questions about student support, your student’s academic learning, and school safety, as well as your perceptions of the school environment. By participating in this survey, you will provide valuable insights that we can use to improve the climate and culture in our schools.
Student Survey
Deadline September 8 through September 11, 2026
The student survey will be given to students in Grades 3 through 12. This is an online survey that covers topics similar to the parent survey.
UPDATE: Student Survey for grades 6-12 will open on September 8, 2026.
The student survey asks about their perceptions of safety, how much they feel listened to and cared about, and the school environment. All of the questions have been reviewed by the District Administration and the District Board of Education.
Student participation in the survey is voluntary; however, we do ask that you notify us if you do not want your child to participate. Kindly do so by Friday, September 4, 2026, at 3:30 p.m. using this Google form - Neelyville R-IV Student Survey Opt-Out Form. A view-only PDF version of the Grades 3-5 Student Survey and Grades 6-12 Student Survey may be viewed on the school website. If you would like a paper copy of the view-only PDF student survey(s) sent home, complete the Printed Survey Form on the back of this document and return to your student’s building office.

A Link to the parent/guardian survey is below, or will be sent home on paper if requested. Students will receive a link to the survey with further directions during the school day. Staff will receive a link in his/her email.

Climate and Culture Survey Student Opt-Out Form - Click to button below to complete the Climate and Culture Survey Student Opt-Out Form. This form only needs to be completed if you do NOT want your student to participate in the Grade-Level Climate and Culture Survey.
